Seniors Case Management

55 years+ or Adults with Disabilities

The Reconnect Case Management program helps seniors 55+ and adults with disabilities navigate the health care and social service systems. Our Case Workers will work with you to plan and coordinate services to help you stay independent and healthy in your home. This can include:


  • Help arranging for health services, home supports, transportation or Meals on Wheels
  • Assistance with filling in forms and completing applications including rental applications, filing your taxes, and completing CPP or other income support application forms.
  • Support group referrals, education and training for family and caregivers. 


We serve clients who live in our service area (see the map below).
