Streets to Homes
16 years+
The Streets to Homes program is a partnership with the City of Toronto in which Reconnect offers support to recently housed individuals 16 years of age and over, who have been identified by the City of Toronto as a part of their Housing First Homeless Initiative. The program serves clients within the Greater Toronto area.
How to Apply
Streets to Homes receives referrals directly from The City of Toronto.
