Aboriginal Legal Services Partnership
Reconnect Community Health Services will accept referrals from Aboriginal Legal Services (ALS) for clients requiring rapid access to the Mental Health and Justice Prevention Case Management Program at Reconnect Community Health Services. The Mental Health and Justice Prevention Program provides case management to people 16+ who struggle with mental illness, addictions, and/or cognitive disabilities, and who are currently involved in the criminal justice system (or are at a high risk of becoming involved).
Eligibility Criteria
- The client is 16 years of age or older
- The client has a serious mental health issue (concurrent/dual diagnosis or cognitive disability included).
- The client can benefit from a community mental health service.
- The client is likely to be safely supported in the community.
- The client has had some involvement in the justice system, which can include current charges, past charges, or release from custody in the past year. Alternatively, if the client is not yet involved in the justice system, there are other risk factors for them becoming involved.
