Things it might be helpful to know about this program:
- The Seniors Case Management program helps seniors 55+ and adults with disabilities navigate the health care and social service systems. Reconnect’s Case Workers work with clients to help them plan and coordinate services to help them stay independent and healthy in their own homes.
- To be eligible for service, an individual must be 55+ years of age, have age-related mental/physical health issues, and be in need of services, supports or resources to manage their day-to-day responsibilities. A person may be eligible under the age of 55 if they have the same needs and have a significant level of physical or mental frailty.
- Service is generally short-term and will end once the client’s goals are met.
- Eligibility for service will not be denied on the basis of language or disability. Supports will be arranged for as needed.
Once in service you can expect the following:
- You will participate in an initial assessment with your case worker that will help to identify your strengths and your needs. Your case worker will help you to establish goals for yourself, which will be the focus of the service.
- Case workers will coordinate your services in partnership with you.
- The case worker will assist you n finding and coordinating services including health services, home supports, transportation, Meals on Wheels, coordinating and advocating for income supports, filing taxes, filling out documents and forms, etc.
- The case worker will discuss with you how crises will be managed and help to create a safety/crisis plan.
- Your status and progress will be documented.
- You will work with your case worker to determine how your significant others are involved in your care. Your caregivers can be referred to a caregiver support group if needed.
- You will be made aware of the agency’s complaint process and how to access it.
- You will be made aware of their right to privacy, how their personal health information is being kept secure, and how and under what circumstances the agency will collect, use or disclose their personal health information.
- The case worker will develop a transition plan with you.
- You may be discharged from service for the following reasons:
- Client choice
- Having achieved your goals
- Being connected with other similar services
- Frequent no-shows or cancellations
- If needed, you can directly contact Reconnect to be re-admitted to service as needed.
