Health Access Program


The Health Access Program is a time-limited case management program focused on system navigation and care coordination. Its goal is to support newly discharged clients to transition back into the community and to reduce hospital readmissions. Clients will receive timely access to community supports, care coordination between new and existing services, and discharge planning once service integration has been secured. Referrals are accepted from hospitals, primary care and community agencies.

Eligibility Criteria

The client must:

  • Be pending discharge from hospital or newly discharged from hospital (one month or less)
  • Have an acute or persistent health condition
  • Need services, supports or resources to live safely in the community
  • Need identification and connection to appropriate community, health, and social services to reduce hospital readmission
  • Be a resident of South Parkdale
  • Not already have access to case management services
