Our Services
Reconnect offers a wide range of community-based services for seniors, caregivers, and people living with mental health and addictions concerns. Whether your goal is to live independently, learn coping skills, or stabilize your health, we have services and programs to help you.
If you are unsure which program might best suit your needs, you can make an appointment to speak to our Intake Department. Our staff will be happy to walk you through your options and what might work best for you.
Community Support for Senior Services
The Adult Day Services program provides supervised social, recreational and therapeutic programming for seniors and adults with disabilities. Through Adult Day Services, people with physical and/or cognitive impairments, including dementia, can access valuable programming that supports their health and overall well-being.
The Assisted Living program provides free on-site personal care and support services for seniors and adults with disabilities. Many of our services are provided in designated Toronto Community Housing buildings. Both short- and long-term supports are offered.
The COPA program helps adults 55+ who struggle with addictions that impact their daily lives.
Crisis Outreach Services for Seniors (COSS) is an on-call mobile crisis service for older adults (65+) with addictions issues or mental illness. The COSS team will meet with the senior in crisis within 24-48 hours, either in their home or in their local community. Short-term crisis response is available 365 days a year from 9am to 5pm.
The Home Help and Respite Care programs help older adults to stay in their homes longer by providing personal care and/or light housekeeping.
The Meals on Wheels program provides home delivery of nutritious hot meals, frozen meals, and produce boxes for seniors and adults with disabilities. We have options for traditional Ukrainian, Italian, and Canadian meals. The food is delivered by staff or volunteers who also check on clients’ safety and well-being.
The Psychogeriatric Assertive Community Treatment Team is a multidisciplinary team that serves older adults experiencing serious mental health concerns, combined with other physical and/or cognitive health problems associated with aging.
The Reconnect Seniors Case Management program helps seniors 55+ and adults with
disabilities navigate the healthcare and social service systems. Our Case Workers work with clients to coordinate services, helping them stay independent and healthy.
The Transportation program provides door-to-door assisted rides to people with mobility issues due to age or disability. Our program is a part of Toronto Ride, a collaborative partnership of thirteen not-for-profit seniors’ services agencies.
Mental Health and Addiction Programs
The Assertive Community Treatment Teams (ACTT) are teams of mental health professionals who work together to provide intensive support to individuals with urgent and complex mental, physical, and emotional needs.
Case management offers one-on-one support from a case manager who helps you identify and work towards your health and life goals.
Central West Concurrent Disorders Specialized Support Services supports people 16+ struggling with both mental illness and addiction (concurrent disorders) living in Etobicoke Rexdale and Dufferin County, including Malton, Brampton, Orangeville and Shelborne.

The Mental Health and Justice Prevention Program provides case management to people 16+ who struggle with mental illness, addictions, and/or cognitive disabilities, and who are currently involved in the criminal justice system (or are at a high risk of becoming involved).
The Primary Care Inter-professional Team is a partnership of healthcare organizations that support family doctors and other primary care providers in the Weston Pelham Park community.
The Psychogeriatric Assertive Community Treatment Team is a multidisciplinary team that serves older adults experiencing serious mental health concerns, combined with other physical and/or cognitive health problems associated with aging.
The Mental Health and Justice Short-Term Crisis Bed program provides 15-30 day stays to people with serious mental illness. Eligible participants are also involved with the judicial system and are currently in crisis. This program serves the Greater Toronto Area (GTA).
Sound Times is a consumer/survivor initiative providing mental health and addiction services, supported by people who have experiential knowledge of the mental health, addiction and criminal justice systems.
In partnership with the City of Toronto, The Streets to Homes program supports recently housed individuals (16+) who have been identified by the City of Toronto as a part of their Housing First Homeless Initiative.
Reconnect provides support to people living in housing provided by our organizational partners. In Peel, we work with Service and Housing in the Province (SHIP). In Toronto, we work with the Canadian Mental Health Association (CMHA) Toronto and Mainstay Housing.
The Transitional Aged Youth (TAY) Case Management Program provides community-based support for young people living with mental illness and addictions challenges, including those transitioning between the youth and adult mental health systems.
Family and Caregiver Support Services (FAME)
Reconnect's FAME Family and Caregiver Support Services offer a range of supports to individuals who are acting as a caregiver for someone who is experiencing a mental health concern. Services include drop-in peer support groups and one-on-one counselling.
Reconnect's FAME Family and Caregiver Support Services - Kids offers counselling services to children (7-12) who have a family member experiencing a mental health concern. Services include virtual and in-person peer support groups, as well as one-on-one counselling.
Reconnect's FAME Supporting Seniors: Caregiver Care helps those who need help understanding and coping with the stress of caring for people who are experiencing Dementia, Alzheimer's, or age-related illness.
Reconnect's FAME Family and Caregiver Support Services - Youth offers counselling services to children (13-17) who have a family member experiencing a mental health concern. Services include virtual and in-person peer support groups, as well as one-on-one counselling.
Health and Wellness Centre
The Reconnect Health and Wellness Centre for Seniors offers social, recreational, and exercise-based programming for older adults and seniors. Groups are offered both virtually and in-person.
Clinical Services
Restorative Art is a form of psychotherapy. These sessions use the creative process to help clients process and articulate thoughts, feelings, and behaviours.
Cognitive Behavioural Therapy (CBT) skills group is a practical, short-term form of psychotherapy that focuses on how thoughts, attitudes, and beliefs affect one’s feelings and actions.
Dialectical Behavioral therapy (DBT) is a form of therapy that aims to teach people how to live in the moment, develop healthy ways to cope with stress, regulate emotions and improve relationships with others.
Mindfulness Cognitive Therapy (MCBT) combines Cognitive Behavioral Therapy techniques with Mindfulness strategies. The goal is to help participants relieve stress by understanding and managing their thoughts and emotions.
Music therapy is a form of therapy that uses music to address the physical, emotional, cognitive, and social needs of participants. A trained music therapist guides the group through various musical techniques and interventions to achieve their therapeutic goals.

Reconnect has been asked to lead a number of system-level projects aimed at supporting the community health sector to improve access, quality, and value of services to clients.
Community Shared Services Collaborative
We assist community health service providers with the end-to-end procurement process. We do not participate in the evaluation process but can provide templates and guidance that will prepare your internal evaluation team for the task.
Performance Improvement
The Department of Performance Improvement uses leading statistical knowledge and methods to support clinical decisions and reporting for the organization and its collaborators.
