Support for Primary Care Providers in Weston Pelham Park
Connections is a partnership between Reconnect Community Health Services, Davenport Perth Neighbourhood and Community Health Centre, the Independence at Home Community Outreach Team, Access Alliance Multicultural Health and Community Services and SCOPE, a virtual health team supporting primary care providers. Our goal is to connect primary care providers serving patients in Weston Pelham Park with specialist and community-based health care services.
Eligibility Criteria
- Youth aged 13-29 living with mental health and substance use challenges who would like to connect with a counsellor.
- Adults who live with mental health issues and need help to get connected with supports and services.
- Individuals who have chronic health issues such as asthma, diabetes or COPD and would like assistance managing their health issues.
- Isolated and fragile older adults who would like to live well and safely in their homes.
- To be eligible for service, patients must live east of Jane/Keele, south of Eglinton, west of Ossington and north of Bloor St.
