Sinai-UHN Geri Hub

Referral for Case Management at Reconnect Community Health Services.

Open to patients of the Sinai-UHN Geri Hub referred by Geriatricians and Clinicians from Sinai Health and UHN Hospitals.


Eligibility Criteria

Patients must:

  • Be a patient of the Sinai-UHN Geri Hub
  • Be aged 65+ or 60+ with cognitive concerns
  • Have mental/cognitive/physical health issues
  • Be in need of services, supports or resources to manage their day-to-day responsibilities.
  • Lives south of Eglinton Ave, north of Lake Ontario, west of Bayview Ave and east of the Humber River.
  • Patient requires:
  • Identification of appropriate community services to better meet their needs and referral support to them
  • Assistance with filling forms and completing applications
  • Advocacy, Navigation and Care Coordination support of health care and social services
