Addictions Case Management

Things it might be helpful to know about this program:

  • The Addictions Case Management Program offers one-on-one addictions counselling, referrals and resources to Reconnect clients who want to make changes in regards to their addictions.
  • To be eligible for service, you must be 15+, currently enrolled in services with Reconnect and have an addictions issue.
  • Eligibility for this service will not be denied on the basis of language or disability. Additional support will be arranged as needed.


Once in service you can expect the following:

  • You will not be judged.
  • The Addictions Support Case Manager will meet with you to find out what your goals are in terms of managing, coping with, or making changes to do with your addictions. They will work with you to create a plan, and then help you work towards your goals. They will provide you with addictions-specific resources and refer you to other addictions-related services, including connecting you with doctors who are addictions specialists if that is what you want.
  • Reconnect’s Addictions Support Case Manager will work with you to problem solve how to live with your addiction and how to deal with your triggers and your emotions around having an addiction.
  • The Addictions Support Case Manager will work collaboratively with you and your primary Case Manager.
  • You can be discharged for the following reasons:
  • Your preference
  • You have achieved all your goals
  • Being connected with other similar services
  • Lack of engagement, meaning you have not met or spoken with our staff about working towards your goals
  • Harm reduction supplies can be provided as needed.
  • Your status and progress will be documented.
  • You will work with our staff to determine how or if your significant others are involved in your care.
  • You will be made aware of Reconnect’s complaint process and how to access it.
  • You will be made aware of your right to privacy, how their personal health information is being kept secure, and how and under what circumstances Reconnect will collect, use or disclose their personal health information.
  • If needed, you can be re-referred by your Case Manager to the Addictions Case Management program after you’ve been discharged.
