COPA

Things it might be helpful to know about this program:

  • COPA is an addictions case management program for older adults 55+ who are experiencing substance use issues.
  • The service helps clients to manage their use of substances safely, to help them maintain their day-to-day responsibilities, and to meet their goals for their own health care needs.
  • To be eligible for service an individual must:
  • Be 55+ and be experiencing substance use issues.
  • Not already be accessing other additions services.
  • Live east of Kipling Ave., west of Victoria Park Ave., and south of Eglinton Ave.
  • Eligibility for service will not be denied on the basis of language or disability. Supports will be arranged for as needed.


Once in service you can expect the following:

  • You will be provided with an assessment and a screener to help you identify your goals and assess which supports would best meet your needs.
  • You will work with COPA Case Managers to help you identify strategies and coping mechanisms that will reduce the harm from continued use of substances or help you to achieve abstinence if that is your goal.
  • Your case manager will discuss with you how crisis will be managed and help you to create a safety plan.
  • You will have access to addictions seniors support groups.
  • You will have access to a concurrent disorders psychiatrist.
  • You will be supported to build connections and resources that will help you maintain a safe living environment.
  • The service is time-limited, meaning that we will help you work towards your goals, typically for approximately one year, and then reassess your needs or support your transition to a longer-term service.
  • Your status and progress will be documented.
  • You and our staff will work together to determine how your significant others are involved in your care.
  • You will be made aware of the agency’s complaint process and how to access it.
  • You will be made aware of your right to privacy, how your personal health information is being kept secure, and how and under what circumstances the agency will collect, use or disclose your personal health information.
  • Clients will be discharged for the following reasons:
  • Client choice
  • When they have completed or neared completion of their goals and been linked to community supports and resources
  • Frequent no-shows or cancellations
  • If needed, you can directly contact Reconnect to be re-admitted to service within a year of discharge.
