Book an appointment
Click the "Get Started" button below to select a time for our Intake Department to call you to discuss your needs, then fill out our secure and easy online form. You can ask your family physician or health care provider to complete a referral for you.
Speak with a Reconnect staff member
After filling out the form, you will receive an email notification and appointment reminder. At the selected time, a staff person will contact you by phone to confidentially discuss your needs and figure out the next steps.
After your first call you may be:
Directly connected to services.
Our staff may take up to a week to put together a care plan that will meet your needs, which they will discuss with you.
