Primary Care Inter-Professional Team

The Primary Care Inter-professional Team is a partnership of health care organizations supporting the family doctors and other primary care providers of Weston Pelham Park, in the west end of Toronto. The team provides access to specialists and community-based health care services for the following health concerns:
- Mental health services.

- Counselling and supports for youth aged 13-29 living with mental health and substance use challenges.

- Chronic health issues like asthma, COPD and diabetes.

- Isolated and fragile older adults who would like to live well and safely in their homes.
