Mental Health and Justice Prevention Case Management
16 years+
Mental Health and Justice Prevention Case Management provides case management services to individuals 16 years or older that have a mental illness (concurrent/dual diagnosis or cognitive disability included) and current involvement in the criminal justice system or who are at immediate risk of involvement with the judicial system. Further eligibility requirements can be found on the referral form. The Prevention program serves the TC LHIN and North Etobicoke.
