Mobile Enhanced Assisted Living Services
Mobile Enhanced Assisted Living Services (MEALS) is a community-based support service for older adults and seniors with complex needs who are able to continue living at home, directing their own care (or via a substitute decision maker) as long as they have access to personal supports on a 24/7 basis. The service provides case management, personal support services such as bathing, medication support, incontinence care, light housekeeping and more.
Eligibility Criteria
- 55+ (may consider younger if deemed appropriate)
- Designated ALC or at-risk of ALC designation
- Valid OHIP
- Require approximately 5 hours of personal support services per week (stand alone or in addition to existing services) in order to safely return home
- Able to remain safely at home between visits
- Home has physical features necessary to enable service to be provided
- Required medical support in place
Clients must NOT require 2-person transfers, trach or tube feeding
