Things it might be helpful to know about this program:

• The Health and Wellness Centre for Seniors provides a wide variety of social, recreational, exercise and meal-based programming for seniors. Services are provided in person at our 2562 Eglinton Ave W site and in seniors apartment buildings within Reconnect’s service area. Services are also provided virtually or over the phone.
• To be eligible for service, an individual must be 55+ years of age.
• In-person services are focused on seniors living within Reconnect’s service area. Virtual services are open to anyone regardless of their location.
• In-person services cost $40 for an annual membership. Virtual services are free.
• To attend in-person services you must be fully vaccinated against covid-19 and comply with all public health guidelines. In-person classes are all pre-registered. Virtual programs are open to all. Many are drop-in.
• Eligibility for service will not be denied on the basis of language or disability. Supports will be arranged for as needed.

Once in service you can expect the following:

• That staff will be on-site from 9am-5pm, Monday to Friday.
• That group sizes will be kept relatively small to facilitate connections between participants and with staff.
• That staff will be able to connect you with other services as needed, such as case management, Meals on Wheels, transportation, home help, or others.
• You will be made aware of the agency’s complaint process and how to access it.
• You will be made aware of your right to privacy, how your personal health information is being kept secure, and how and under what circumstances the agency will collect, use or disclose your personal health information.
• You may be discharged from service for the following reasons:
• Your choice
• Actions that cause a provable risk to others
• If your health needs become more than can be managed at the Health and Wellness Centre
