Mount Sinai Community Referrals
Reconnect and Mount Sinai Hospital have partnered to provide quick and easy access to seniors services and mental health services in the community for Mount Sinai patients leaving hospital.
Eligibility Criteria
Services for Seniors:
- The patient must be 55+
- Patient should be inpatient in hospital or a long-standing hospital outpatient.
- The patient must live within Reconnect’s catchment area for the desired service. Patients requiring case management must live south of Lawrence Ave, west of Bayview Ave, north of Lake Ontario and East of Humber River. For any other desired service, patients must live within Reconnects catchment area, which is west of Spadina Ave, north of Bloor St W, east of the Humber River and south of Lawrence. Please see program descriptions for further program-specific eligibility criteria.
- Patients must have a residence.
Mental Health and Addictions Services:
- The patient must present with persistent and complex mental health issues. Serious mental health issues may be defined by the duration of the illness and the level of disability it has caused. Primary diagnoses may be schizophrenia, schizo-affective disorder, bi-polar disorder and/or chronic depression. Patients who have a secondary diagnosis such as mild developmental disability, brain injury, and/or substance use problems and problems regulating emotions are eligible. Patients with only addictions issues are not eligible.
- The patient requires case management for unmet community health care needs. This includes system navigation and support to stabilize mental health symptoms.
- Patient should be inpatient in hospital or a long-standing hospital outpatient.
- The patient and/or their substitute decision maker are willing to accept services.
- Patients must have a residence.
- The patient must live within Reconnect’s MHA services catchment area, i.e. generally west of Yonge St, south of Eglinton, north of Lake Ontario and east of Highway 427, although some programs catchment areas will differ.
- The patient must be at least 15 years of age.
