The ACT and Multidisciplinary teams include a mix of the following care providers: psychiatrist, nurse, case manager, and depending on the team can also include an occupational therapist, social worker, and addictions worker.
The Addictions Case Management Program offers one-on-one addictions counselling, referrals and resources to Reconnect clients who want to make changes in regards to their addictions.
The Adult Day Program offers social, recreational and therapeutic group programming in a secured environment. Personal support care is available for incontinence care and feeding if required. Medication support can be provided as well.
The Assisted Living program provides free on-site personal care and support services for seniors and adults with disabilities. Many of our services are provided in designated Toronto Community Housing buildings.
Behavioural Therapy works with people to identify and change behaviours that can be problematic in achieving a safe life and encourages those that will help them meet their everyday needs.
Case management offers one-to-one supports by a case manager. The case manager will
support you in identifying goals that will help you to meet your health care needs.
COPA is an addictions case management program for older adults 55+ who are experiencing substance use issues.
COSS is an on-call mobile crisis intervention and outreach service for seniors that provides short-term support.
DBT is a form of therapy that aims to teach people how to live in the moment, develop healthy ways to cope with stress, regulate emotions and improve relationships with others.
FAME Family and Caregiver Support Services offer a range of supports to individuals who are
acting as a caregiver for someone who has a mental health concern.
The FAME Supporting Seniors: Caregiver Care program is a group-based service that provides support to individuals who are caregiving for someone experiencing aging-related challenges.
The Reconnect Health and Wellness Centre for Seniors offers social, recreational, and exercise-based programming for older adults and seniors. Groups are offered both virtually and in-person.
The Home Help and Respite Care programs help older adults to stay in their homes longer by providing personal care and/or light housekeeping.
The Meals on Wheels program provides home delivery of nutritious meals to seniors and adults with disabilities.
MHJP provides case management to individuals that have a mental illness and current involvement in the criminal justice system or who are at immediate risk of involvement with the judicial system.
Reconnect’s Mindfulness group helps participants to address symptoms of depression and anxiety and reduce stress.
The Occupational Therapist will look at your physical health and evaluate what you might need in your home to live in it safely, and what activities you can do to improve your health.

The Reconnect Seniors Case Management program helps seniors 55+ and adults with disabilities navigate the health care and social service systems.
The STCB program provides short stay residential support in the community to individuals who are living with serious mental illness, are involved with the judicial system, and are in crisis.
The Transportation Program provides rides to frail seniors and adults with disabilities.
EHTS is a 90-day program designed to provide home supports and personal care to people returning home from being admitted to hospital.
