HEART@Home Partnership

Humber River Hospital's Elderly Assess & Restore Team (HEART) is a mobile healthcare team that sees patients, 65 years of age and older, who have been admitted to the hospital. HEART assesses a patient’s risk of losing their strength, functional mobility*, and independence, and provides them with treatment and support. The team’s goal is to help rebuild their strength, increase their independence, and build their confidence to return home. Reconnect supports HEART patients through supports in the community. Referrals are accepted from the Humber River Hospital HEART team.
