Case Management

Reconnect's Case Management program works with people who are experiencing mental health concerns and who require support to manage their symptoms, gain independence, and work towards achieving their goals for themselves.


Case Managers will assist you in developing strategies to maintain  self-care, symptom management, crisis management an developing a support network, as well as life skills including budgeting and grocery shopping. The case manager will meet with you in person or virtually and connect you with what we can offer for your particular need.


Eligibility Criteria: To be eligible for service, an individual must:

  • be at least 15 years of age
  • be experiencing a serious mental health issue or be someone who's mental health is impacting their ability to manage their healthcare needs
  • have goals they want to work towards
  • for those accessing virtual services, you will also require a device such as a smartphone, tablet or laptop to connect with our staff. You will also need to be comfortable meeting with our staff in person if that is deemed necessary for your support plan. 



For those who are experiencing mental health concerns but don’t require the support to coordinate other services, we have a number of clinical group programs including Cognitive Behavioural Therapy, Dialectical Behaviour Therapy, Mindfulness-Based Cognitive Therapy, as well as Art and Music Therapy that you can access as well. 

This program serves those living in the area east of Highway 427, west of Yonge St., south of Eglinton Ave., and north of Lake Ontario.
