Integrated Transitional Services - Harbour Lights is a TC LHIN initiative created to support the transition from hospital to community for two patient groups: those whose bariatric needs are presenting as barriers to a transition to a community setting, and those whose mental health and addictions needs are presenting as barriers to a community setting.
