Adult Day Services

Things it might be helpful to know about this program:

  • The Adult Day Program offers social, recreational and therapeutic group programming in a secured environment. Personal support care is available for incontinence care and feeding if required. Medication support can be provided as well.
  • To be eligible for service, an individual:
  • Must be assessed by a Case Worker.
  • Must have a need for the service, require a supervised environment and be unable to benefit from other community activities due to cognitive or physical limitations.
  • Must be comfortable being part of a program with other people with similar health care issues and must be able to be supported as part of a group (meaning they do not require frequent one-to-one support).
  • Must live within Reconnect’s Transportation service catchment area if they desire transportation services to and from the program.
  • Must be able to bear weight and at most require the assistance of no more than one person to move in and out of chairs or around the program space.
  • Eligibility for service will not be denied on the basis of language or disability. Supports will be arranged for as needed.


Once in service you can expect the following:

  • Clients and caregivers will participate in an initial assessment with staff to develop a care plan based on clients’ needs and goals while at the Adult Day Program.
  • To receive a service agreement detailing costs, days of service, eligibility criteria, reasons for discharge and behavioural expectations.
  • The client’s status and progress will be documented.
  • The client will be offered exercise and movement at each visit, as well as the opportunity to participate in a variety of programming geared towards the client’s abilities.
  • Clients will receive breakfast, lunch and snack daily.
  • Caregivers will be provided with regular updates of how their loved one is doing at the program.
  • An annual reassessment and review of the client’s goals by staff with caregivers.
  • Clients and caregivers will be made aware of the agency’s complaint process and how to access it.
  • Clients and caregivers will be made aware of the client’s right to privacy, how their personal health information is being kept secure, and how and under what circumstances the agency will collect, use or disclose their personal health information.
  • Clients can be discharged for the following reasons:
  • Client/caregiver choice
  • Achievement of goals
  • Being connected with other similar services
  • Frequent no-shows or cancellations
