Enhanced In-Home Transitional Services

(EHTS)

Personal support care for designated ALC patients.

Enhanced In-Home Transitional Services (EHTS) is a community-based support service for older adults and seniors who are designated ALC at Toronto-area hospitals and who are able to be safely transferred home with access to personal care support. 


The service provides case management and personal support services such as bathing, medication support, incontinence care and light housekeeping for up to 90 days.

Eligibility Criteria

 

Clients must:

  • Be designated ALC
  • Be age 55+
  • Have a discharge destination
  • Have all equipment necessary in the home to live there safely
  • Be able to manage with HCCSS services after 90 days
  • Test negative for Covid-19 upon discharge from the hospital
  • Be able to feed themselves
  • Be able to take their own medication without assistance
  • Be able to self-direct in and out catheterization (if required)
  • Live between Yonge St and the Humber River, and between Lawrence Ave and Lake Ontario
  • Clients who are on LTC and LTC Crisis waitlists are eligible for service.
  • Clients requiring 24/7 care are NOT eligible. Those requiring 24/7 care for a short period of time (up to a month) may be considered on a case-by-case basis if it can be demonstrated that the high level of service will not be required beyond the specified timeframe.
